We are here to serve you and this website is set up to aid you in a successful experience with your child’s Athletic Journey while at Calvary Chapel Christian School. From forms that your child needs to fill out for sports participation to directions to your child’s game and the most recent scores, you will be able to find it ALL here! Athlete Checklist 2016-2017 Please refer to following checklist so that you will understand exactly what is required to participate in the CCCS High School Athletics Program.
Guidelines for Participants
For students to be eligible to participate in the Calvary Chapel Christian School’s Athletic Program they must meet the following guidelines. All forms must be must be submitted to the Athletic Office prior to participation for any sport in the summer or regular season.
Incomplete packets will NOT be accepted and your student will NOT be able to participate.
o Prayerful consideration on the part of you and your child to ask the Lord’s guidance in regards to participation.
o Physical Evaluation form completed by a licensed physician – signed and dated. Your child will not be permitted to participate if they do not have a current physical evaluation on file before the first day of practice. Please note the portion of this form that must be filled out by Parent/Athlete with signatures. This must be turned in to the Athletic Office. (Physicals are good for one calendar year).
o Handbook Agreement Form Parent and Athlete signatures are required. This is acknowledgement and agreement to the schools policies and procedures that are in the handbook, please access the handbook from the school web-site under parent resources.
o Insurance / Transportation Waiver Parent Signatures are required so CCCS can transport your child. Each student/athlete must have personal health insurance.
o First Aid Waiver Parent and Athlete signatures are required, giving CCCS consent to treat your child in case of injury.
o CIF Code of Ethics Form Parent and Athlete signature required agreeing to the standards of CIF sportsmanship, rules and guidelines.
o CIF Concussion Information Sheet Parent and Athlete signatures required agreeing that you have received this important information.
o Sudden Cardiac Arrest Information Sheet Parent and Athlete signatures required agreeing that you have received important information regarding SCA.
The above (7) forms are due before the athlete can participate and are good for one school year of sports.
o UNIVERSAL ATHLETIC FEE REGISTRATION FORM Be sure to enroll in this discount program. No contracts for this program will be accepted after August 8, 2016.

FALL SPORTS CONTRACTS DUE BEFORE AUGUST 8, 2016
o Girls Volleyball: Contracts Due August 1, 2016 must be turned in before eligible to try out. (8/1-5)
o Football: Contracts Due August 1, 2016
o Cross Country: Contracts Due August 8, 2016
o Contracts with above mentioned forms are due before August 8, 2016. Late fees apply if contract is not received by due date.
o Fall Contracts – you can click on your sport (above) to print a Fall Sports Contract, or you can obtain them from the Athletic Office. These contracts officially enroll your student in Fall Sports.
o WINTER SPORTS CONTRACTS DUE BEFORE NOVEMBER 7, 2016
o Contracts with above mentioned forms are due before November 7, 2016. Late fees apply if contract is not received by due date.
o Winter Contracts – you can click on your sport (above) to print a Winter Sports Contract, or you can obtain them from the Athletic Office. These contracts officially enroll your student in Winter Sports.
o SPRING SPORTS CONTRACTS DUE BEFORE FEBRUARY 6, 2017
(These contracts will become available in January, 2017)
